About Glassmoon services
We specialise in providing human-centred, specialist services, supporting young adults and adults with autism and/or learning disabilities to live their best lives, in their own homes, within the community. Where everyone can thrive with meaning and fulfilment, creating a positive social impact every day.

Experience Sensitive and Human Rights Led

Our model is a trauma-aware, experience sensitive and non-pathologising approach that protects people’s human rights and promotes social inclusion and citizenship.

We are commissioned to support and transition people out of long-term institutionalised units, and we work proactively to prevent unnecessary admissions to secure hospitals.

Team-Around-the-Person

Our daily practice is based on a Team-Around-the-Person approach, and we work as part of a broader Multi-Disciplinary Team (MDT) with health professionals and Social Care specialists. Our care and support are created on a bespoke basis to reflect individual needs.

Our approach reflects the high-acuity specialist services required for the person being supported to live in their home and not spend their life in a hospital setting.

As we work to change the system for everyone, we can change the world for one person at a time.
